instead of having a separate tab called signatures, add a menu item under action called Signature that allows them to add a signature to documents

Currently there are tabs for signature in the demographics and in care plan. Remove those and instead, after they upload a document, provide the ability to add a signature for the document uploaded. This will be in the client document and the case record document area
