Adds the OutputTransformer product: the Transformer namespace and the Module
protocol that post-processing steps implement. Concrete transformers now ship
with the libraries that own their vocabulary (MachOSwiftSection for the Swift
comment kinds, MachOObjCSection for the ObjC ones), all extending this same
namespace.
