Add is_mcp-gated MCP upstream request tracing

Trace requests proxied to mcp locations (McpHttp1 over the h1 pool):
reached/sending, OK+status, FAILED, and UNAVAILABLE. Gated behind a new
is_mcp flag on Http1ContentSource so only mcp locations log — regular
http/http2/https locations stay silent.
