Hide empty Grok Think rows and drop server-search tool echoes.

• Server-side web_search / x_search still run on the CLI proxy. Results stay in packed tco_* replay.
• Empty tco_* reasoning items no longer each paint a Think row.
• Grok sometimes echoes the same X/web search as a client custom_tool_call (xs_call-* / ws_call-*, often named x_keyword_search). Those are no longer forwarded to DSH, so the agent does not report unknown tool.
