small fix for small issues
Added 'endpoint_hash' => $endpointHash to the update values — this is the actual fix. Without it, any new subscription (fresh install, new device, or after a VAPID key change) fails at the DB level because the column is NOT NULL with no default.

Replaced the duplicate $request->string('endpoint')->toString() call in the lookup with the already-computed $endpoint variable — minor cleanup.
