Fixes

  • Convergence metrics WARN check was a no-op — validator now detects placeholder values instead of just checking for section header presence
  • Topic extraction was greedy — appendToIndex no longer leaks [CORRECTED iter-N] annotations into INDEX.md topics
  • No build-time validation of validator transitions — Makefile and build.ps1 now cross-check validate-plan.mjs VALID_TRANSITIONS against SKILL.md
  • Test count stale — CLAUDE.md and README.md updated (102 tests)
  • convergence-metrics.md missing from file trees — added to both CLAUDE.md and README.md

2 new tests (102 total). All pass.
