Fixed

  • stripHeader H1 injection — stripHeader() in bootstrap.mjs could inject a stale H1 heading into consolidated files
  • verification.md template — corrected bootstrap template formatting
  • INDEX.md pipe escaping — pipe characters now properly escaped in table entries
  • Validator numbered findings — validate-plan.mjs now correctly parses numbered findings lists
  • Test counts and README project tree — CLAUDE.md and README.md updated
