  1. Fixed renaming or replacing a callout type so heading and inline callouts are updated too, instead of being left behind with a dead id.
  2. Fixed "Delete (convert to plain text)" so it also converts heading and inline callouts, instead of leaving them orphaned when the type is removed.
  3. Fixed heading callouts to preserve title spacing correctly.
