Fixed the delete/replace confirmation window (and the plain confirm dialog) showing an empty, ruled header band when it had no title.
Fixed the "replace with" picker auto-selecting a callout type for you; it no longer pre-selects anything, and the confirm button stays disabled until you actually choose one.
