Changes:

• Add multi-harness Codexplain adapter support from PR #15.
• Add GitHub star onboarding during npm postinstall, first project install, and first settings run.
• Support terminal-only star flow via authenticated GitHub CLI without opening a browser.
• Support agent/non-interactive consent with CODEXPLAIN_STAR=yes or CODEXPLAIN_AGENT_INSTALL=1.

Validation:

• cargo fmt --check
• cargo test (128 passed)
• npm pack --dry-run with isolated npm cache
