• README images blocked in markdown preview: skipHtml dropped raw <img> tags and relative paths couldn't resolve against the webview origin. The preview now rewrites <img> to markdown image syntax, reads relative images via fs_read_file_bytes, and inlines them as data: URLs (remote https: images pass through). CSP img-src widened to allow https: and http://localhost:*.
• List item rendered as a text block: wrapAsciiArt misclassified markdown list items containing box-drawing or |-bracketed content as ASCII art and wrapped them in code fences. List items are now excluded from art detection.
• App froze during LLM streaming (terminal lagged too): stripLeakedTokens + Streamdown re-parsed the full accumulated text on every streamed token (O(n²)). Streamed text now renders at a throttled ~12fps with an exact flush on stop, and the copy button computes cleaned text lazily on click instead of per render.
• Chats with an ignored tool approval were unrecoverable: sessions whose last run ended on an unapproved tool call opened blank and couldn't be re-selected. hydrateSessions now seeds the restored active session's history before flipping the active id, and loaded history strips incomplete approval-requested tool parts at the part level so the session recovers instead of pinning awaiting-approval.
