Fixes HTML-entity escaping of element names (apostrophes, ampersands, quotes).

  • Names like "The Kid's Family" were written into notes HTML-escaped (The Kid's Family), breaking [[link]] file lookups and storing escaped names via the API.
  • Root fix: note templates no longer HTML-escape values; existing escaped notes are decoded on read, so old vaults self-repair on their next save/upload.

No settings or format changes.
