- Prevent compiler from overoptimizing hash bucket logic in envs where OCF_COND_RESCHED is a no-op
- Limit read lock holders to prevent access counter overflow
- Prevent failing request map allocation for large I/Os (>1MiB) in fragmented memory conditions
- Fix bisect generator logic to avoid undefined behavior from __builtin_clz(0)
- A few minor fixes
