Extended test suite from 26 to 46 tests for better coverage.

• test_errors.sh - 10 tests for error paths and edge cases
• test_options.sh - 10 tests for options and command aliases
• tests/run_all.sh - unified test runner with summary
• tests/fixtures/errors.md - edge case test fixture

Test Coverage Now Includes

• Missing/multiple input files
• Empty files and files without headers
• Unclosed code blocks, indented fences
• Non-standard headers (no space after #)
• Multi-level boundary violations
• Version output, custom backup suffix
• Command aliases (up/down)
• Bundled options, quiet/verbose modes
