  and setup.sh. All tests pass under the new names.

Removed

• --profile=mcptap flag — the obsolete --profile=mcptap flag has been
  removed from all Codex CLI startup commands in setup.sh, docs/FEATURES.md,
  and the README example for the file-block library.

Fixed

• README cleanup — removed duplicated "Requirements" and "Health endpoint"
  sections from the README; the per-session directory description and
  LD_PRELOAD env-var table have been consolidated. A new LD_PRELOAD
  file-block library env-var table (MCPTAP_FB_DIR, MCPTAP_FB_INTERPRETERS,
  MCPTAP_FB_ESCALATORS, MCPTAP_FB_DISABLE_ESCALATOR_CHECK) replaces
  scattered references with a single, structured reference.
