Survey Mode Now Supported

Enhancement: adding support for entries that include commas.
Enhancement: removing hardcoded values so the module's action tag can be fully configurable.
Enhancement: the module now supports adding Flex Fields to surveys; plus more code clean up.
Bug Fix: the flex field was being added next to the host text field by mistake, making it seem as if two text fields where on the same field. The host text field is now being hidden by the module and the ad hoc fields are the only ones being displayed as expected.
