Codifies the issue/milestone realignment done today: release-please makes version numbers outputs of commit history, so milestones are now thematic buckets (version names reserved for deliberately gated majors), and issues associate with releases via commit references rather than milestone names. Adds a Versioning, Releases, and Milestones section to CONTRIBUTING.md so the convention outlives this cleanup.
