fix: prevent mutex map leak on try* for KeyedMutex and AsyncKeyedMutex

Previously, tryLock/tryLockShared could create per-key mutex entries and return
early without decrementing the reference count, leaving stale entries in the
internal map (memory leak). Fixes ensure ref counts are incremented when a
mutex is tracked and always decremented on failure or after task completion.
