Refunds, webhook management and the missing read endpoints.

• refund_create — refund a received Pix charge. Gated behind cash-out, like withdrawals: unavailable on the hosted server, available in the local app.
• Webhook management (8 tools): create, list, get, update, delete, rotate secret, delivery count and delivery detail.
• Reports: bank statements, pending deposits and account summary.
• account_pix_keys and callbacks_resend_webhook.

29 to 45 tools. The bundled .mcpb now reports the correct version (the previous one shipped a stale package.json pinned at 0.4.0).
