TASK-2000 bumped ToolSurfaceVersion 0.8->0.9 (summary-shaped item list) in parallel with TASK-2005's v0.8 doc sync + drift guard; the two merged cleanly as text but left instructions.md/README at v0.8, tripping the guard. Bump both to v0.9 and note the change in the changelog line.

Follow-up to the parallel merges of TASK-2000 (bumped
ToolSurfaceVersion0.8→0.9) and TASK-2005 (v0.8 doc sync + drift guard). The two merged cleanly as text but leftinstructions.mdand README at v0.8, tripping the drift guard onmain. This syncs the three version strings to v0.9 and adds a v0.9 changelog note (summary-shapedpad_item.list). Action lists unchanged.go test ./internal/mcp/...green.
