fix(mcp): prioritize npm package over copy-paste in all tool responses

- SERVER_INSTRUCTIONS now mandates npm install before using any component
- list_components and get_component descriptions point to npm, not copy-paste
- get_component response leads with npm install + import instructions
- Removed inline source code sections that pushed AI toward copy-paste
- Source is available via README/types only — import from npm package
