Adds a reconciliation_baseline trigger for legacy live baseline enrollment. This trigger always runs the independent multi-review path even when the primary reviewer is highly confident, requires a SHA-256 candidate binding, and returns that binding in the audit result. Prompts explicitly forbid treating approval as an order or state-transition authority. Adds focused regression tests and operational documentation.
