Fix: Settlement list now groups by type

When you have hundreds or thousands of settlements, the settlement list was rendering one entry per settlement. At 3,000+ settlements, this meant thousands of identical lines for a screen reader to navigate through.

Settlements are now grouped by tier and specialization with a count. A realm with 3,124 settlements now shows roughly 15 lines instead of 3,124.

Example: instead of 100 identical "Capital (Craft District)" entries, you see:
100 Capitals (Craft District)

No gameplay changes. Saves are fully compatible.
