Fixes for multi-vendor setups.

• A role on a named backend must now name a model of its own (in the role or
  LLM_BACKEND_<NAME>_MODEL). Without one the role is no longer used: the main generator
  falls back to the installation provider and says so, other roles are skipped. Previously
  the call went out with the installation's model — one vendor's model on another vendor's
  endpoint, which fails and costs an attempt.
• The LLM backend '<name>' log line no longer names a model the call does not use.
• The run summary's tail (incremental coverage, cost, open threads) is a list again, not one
  glued paragraph.

Every platform under two names: reviewgate-2.0.5-* pins the version, reviewgate-latest-*
