feat: follow Claude Code's own report of who holds the keyboard (#103)

Focus moves between the prompt box and the terminal were decided by reading
the screen, which misses views it doesn't know: /plugins and its submenus
bounced focus back to the box about once a second.

Every session now loads a bundled mod (resources/term-bridge, via
--plugin-dir) that tells the app when a dialog, menu or other view holds
Claude Code's keys and when the plain input line is back, plus slash-command
start/end. Focus follows that; screen reading stays the fallback for
sessions without the mod. The mod's events and the app's focus moves are
logged side by side to ui-bridge.log in userData.
