errors: use explicit unicode theme in `DiagnosticExt::render` 1cda8e9

• Replace auto-detected GraphicalReportHandler::new() with
  new_themed() using GraphicalTheme::unicode() so diagnostic
  output renders with unicode characters regardless of terminal
  detection (e.g. inside Docker)
• Respect the NO_COLOR env var per the no-color.org convention
