Popup contact form using Ajax and style in CSS3

This another contact form that is easy to use and is customised in CSS3 and Ajax,
This form will popup on click of link/button and has a dropdown feature for enquiries ect,
I will also provide basic css for the demo page and I'll include css for a button, this will also be CSS3 and works on all browsers.
