- Review diffs use stable file paths and headers instead of user-specific Git formatting or external diff commands.
- A nonempty review diff with unreadable file entries stops approval instead of being treated as an empty change.
- Planning reuses the registered Plan identity when it is missing from the document. Conflicting identities stop the operation without changing your files.
