Fixed

  • Plan Review that runs out of revisions no longer parks approved work: the bypass is recorded as an audited human decision, and the scheduler plus deferred workflow continuations wake up right away.
  • Dependency changes keep the durable replan signal, so a reseeded plan stays recoverable rather than silently stranding.
  • Mission default merge strategies now integrate through a dedicated mission integration branch, so default-branch landings still pass the normal manual release gate.
  • Stale plan approvals are blocked, with bounded lifecycle locks, planning-episode evidence, serialized approvals, and convergent planning and review ledgers.
  • Voice dictation session create, transcription, and cleanup all use the selected project's identity instead of leaking across projects.
  • Planning finalization no longer crashes after writing the task prompt.
  • Task Detail Activity thinking blocks open by default and keep per-block collapse state while streamed reasoning arrives.
  • Planning gathers repository evidence before drafting, Plan Review batches independently discoverable blockers into one pass, and code review traces changed invariants through consumers and tests.
