- Plan Review that runs out of revisions no longer parks approved work: the bypass is recorded as an audited human decision, and the scheduler plus deferred workflow continuations wake up right away.
- Dependency changes keep the durable replan signal, so a reseeded plan stays recoverable rather than silently stranding.
- Mission default merge strategies now integrate through a dedicated mission integration branch, so default-branch landings still pass the normal manual release gate.
- Stale plan approvals are blocked, with bounded lifecycle locks, planning-episode evidence, serialized approvals, and convergent planning and review ledgers.
- Voice dictation session create, transcription, and cleanup all use the selected project's identity instead of leaking across projects.
- Planning finalization no longer crashes after writing the task prompt.
