Fixed
- Approved tasks no longer become permanently stuck when merge review proof is missing: proof is enforced before dispatch and at the sink, bypass is limited to audited humans, and reconciliation is audited.
- Inline review fixes no longer leave an approved task unable to merge — verified review identity is re-captured and stale singular content is rerouted out of merge admission.
- A merge that already landed now finalizes when the task row has no steps, instead of silently never emitting a merged event.
- Interrupted merges from pre-upgrade worktree layouts now resume without dirty-checkout refusals.
- Finished worktrees holding regenerable build and dependency output are cleaned up; anything not on the allowlist stays behind the landing-proof gate.
- Your selected Claude account now takes precedence over a leftover legacy Anthropic sign-in.
- Adding a credential instance no longer creates a duplicate OAuth account.
- OpenAI Codex sign-in works on remote dashboards (device code by default, explicit browser override), and stalled logins no longer hang in the background.
