Fixed

• The Approve button on board and list cards no longer does nothing after a page reload.
• The review bypass now reaches any blocking gate, not just a failed one, and the merge blocker names the gate at fault.
• Review gates archived as collateral by another gate's remediation are restored on startup and during maintenance, so blocked cards stay recoverable. No verdict is ever fabricated, and operator waivers, workspace cards, paused cards, and live sessions are left alone.
• A review remediation now archives only the gate it is remediating instead of every failed gate.
• Restarting an operator review starts the gate's revision budget fresh instead of inheriting what the previous episode spent.
• Archived pre-merge review failures are recoverable again through operator bypass.
• A verdict rescued from a malformed reviewer reply can no longer be downgraded to an approval — an empty finding list reads as unknown, not as nothing blocking.
• Stale review approvals no longer leave merge cards permanently failed; the outdated review lane is routed back to current content.
• A task being planned is no longer read as abandoned work and re-dispatched mid-planning.
• Planning retries preserve annotated plan steps and manual approval.
