World difficulty, fixed and easier to set

• Pick the difficulty (combat, death penalty, resources, raids, portals) right when you create a server, so a new world starts at the difficulty you chose.
• Fixed a bug where changing a world's difficulty and pressing Save Config could quietly fail to save, or reset it back to normal. Your choice now sticks. Choosing Normal for everything still clears it.

Possibly outdated column

• Added a one week grace period. A mod is only flagged as possibly outdated once the Valheim update it predates is at least a week old, so the whole list no longer turns yellow the day a patch lands.
