Fix remote recovery of pending agent requests - #391

- Bugfix: persist open approval and user-input requests with thread runtime snapshots so remote clients can recover missed prompts.
- Retire persisted request state when requests resolve or turns complete, preventing stale recovery.
- Keep recovery-only request items out of the visible chat transcript.
- Add coverage for persistence, pagination, recovery, retirement, and transcript filtering.
