Indexer: ledger reorg handling

Area: apps/s03-indexer/src/mappings/**

Goal: Ensure reorged ledgers do not leave stale entities.

Tasks:

• Add fixtures for a reorged block sequence.
• Assert affected entities are rolled back or rewritten.
• Assert deterministic final state.

Acceptance:

• No live chain required.
• bun run --cwd apps/s03-indexer test passes.
