Fixes

• An exception thrown by the user's right-hand side, Jacobian or callback reaches the caller without PETSc also printing a traceback to stderr, on solve, the integrator and the adjoint. A zero pivot or overflow still prints one when petsc_options asks PETSc to raise (#41).
