⚠ BREAKING CHANGES

• mcp: a refusing MCP tool result, including tracedecay tool --json output, carries its typed problem record only at structuredContent.problem; the top-level problem member is removed.
• query: code_declaration, code_references, and code_type_definition refuse an unheld pinned generation as not_found_or_not_authorized (retry never) instead of answering unavailable.
• sessions: sessions import schedules historical catch-up and finishes as partial coverage with one deferred unit per store scope; it no longer waits for the pass or reports completed/timed_out. An empty session search during historical convergence answers stale instead of complete_zero.
• mcp: tracedecay tool refusal stderr is <tool> refused the request (<code>): <message>, and argument errors are tool_arguments_invalid invalid requests with a --json problem record. Retained-surface argument refusals answer JSON-RPC -32602 with typed data instead of -32603 with cli_fallback. Unknown ids, foreign scopes, invalid advisory document URIs, and an empty test-run history answer not_found_or_not_authorized / invalid_request with retry: never instead of a retryable unavailable.
