⚠ BREAKING CHANGES

• remote: RemoteAuthorityUnavailableReasonV1 (and the dashboard RemoteAuthorityMissingReasonV1) gain writer_authority_unpublished, and a never-published authority now reports that reason instead of placement_unknown. The TypeScript SDK moves to 0.12.0.
• hook admission ledgers written before the group-commit log, and pre-ledger pending-work spools, are no longer imported; the daemon refuses them until tracedecay wipe --stale --yes resets them. A stale workflow placement digest is answered as a conflict with a workflow_placement_digest_stale detail instead of invalid_request, and conflict problems may carry a detail. The TypeScript SDK moves to 0.12.0.
