Index entries stay one line; detail moves to ~/.dsh/memory/topics/.md, read on demand via the new memory_topic tool (five actions). Bounded reads (400-line window + 8KB cap), destructive read-before-write gate, scanBulk subset, pointer preservation in compact/overflow/GUIDANCE, orphan visibility in /memory topics and the settings panel. 400 tests green; live-verified end-to-end (fork wrote a topic file + pointer entry in one pass).
