Implementation-review fixes: paging opens the gate for files larger than one read window (B1), continuation hints advance across pages (B2), evidence freshness re-closes the gate after background appends (A1), the review fork self-provides the topic schema (A2), plus null-safe validation, limit clamp, and orphan suffix-collision fix. 410 tests green.
