Fixes and improvements

Resolve preview iframe document at check time. The prepublish/highlighting callbacks previously captured the preview iframe's document once during initialization and relied on a 10-second polling loop to find it; navigating afterward could leave the callbacks pointing at a stale or blank document, causing an intermittently missing "Run content check" button and prepublish checks that found no content or errors. The document is now resolved at call time instead of cached, so callbacks always see the current preview.
