Fix stall recovery escalation and stop stale state writes

scheduleNextStep now checks if stall persists, not just heartbeat
timeout, so recovery properly escalates through refresh/toggle/alert
instead of looping at retryCurrent forever
Skip state.json writes during confirmed stalls to avoid overwriting
last good position with frozen currentTime
