Exam Option added StethIOExamOption following properties

examType, audioSampleType, heartMinimumGain , lungTargetLevel, heartTargetLevel.

StethIOExamOption() for default value

Removed setExamType & setAudioSampleType and use StethIOExamOption

StethIOExamOption.init(examType: StethIOManager.ExamType = .heart, audioSampleType: StethIOManager.AudioSampleOutputType = .rawSamples, heartMinimumGain: Float = 2.0, lungTargetLevel: Float = 0.1 , heartTargetLevel: Float = 2.0)
