Optional Ctrl-click folder selection

• Alt-click remains the default way to select folders without expanding or collapsing them.
• Add an opt-in "Select folders with Ctrl-click" setting, disabled by default. Ctrl-click adds or removes folders from the selection without toggling their expanded state, including clicks on disclosure arrows.
• Keep Alt-click available when the setting is enabled; Ctrl+Shift-click uses range selection.
• Save the preference and apply setting changes immediately. Existing settings keep their VS Code executable path.
• Correct the README's selection instructions and explain the new option.

Validation: TypeScript build, ESLint, smoke tests (settings persistence, existing menu/launch behavior, 73 language fallbacks), and 18 browser checks using Obsidian 1.14.4's native selection handler.
