run_skillreceipt authority already rejects fabricated receipt IDs, but the defense-in-depth hook that should stop an assistant from acting on a fabricated background notification cannot reliably correlate live work with the current orchestrator or recognize a realistic fabricated turn. Separately, exceptional unwind can permanently orphan completion state either before drafting in_in_flightor after drafting but before middleware publication in_drafts.Closes #4579
