Fixed

• Folia safety — TPA now captures the target location on the target's entity scheduler and starts the requester teleport on the requester's scheduler.
• Folia safety — shared homes, warps, back locations, and pending teleport state use concurrent collections; persistence writes are serialized.
• Teleport callbacks now resume on the teleported entity's scheduler before accessing Bukkit player state.
• Back locations are recorded only after an async teleport succeeds.
• TPA race — simultaneous requests to the same target now atomically allow only one pending request.
