v1.1.0: Claude Desktop support + BOM fix

New: Configure Claude Desktop app support via the 3P gateway config.

Fix: BOM-less UTF-8 for all JSON writers. PowerShell 5.1 was adding a BOM that broke the Claude Desktop app's JSON parser, causing the 3P config to be silently ignored.

Fix: Multiple Codex config issues (wire_api removal, auth_mode gate, env_key resolution, base URL format).
