v0.4.11 — OpenCode resolve timeout protection

Highlights

• Prevents OpenCode plugin runtime hooks from blocking indefinitely before provider requests.
• Adds LARVA_OPENCODE_RESOLVE_TIMEOUT_MS with a 5000ms default and invalid/zero/negative fallback handling.
• Timed-out runtime resolves now degrade to stale last-known-good prompts or fail-closed prompts without leaking [larva:<id>] placeholders.
• Includes packaged plugin timeout behavior in wheel/sdist artifacts.
