Merge with Rebase

This change merges W3DShaderManager, Smudge, W3DSmudge.

Generals gets the following things

  • Smudge (Heat Haze) code (unused)
  • Flat Terrain Shader, Flat Shroud Shader (currently or always unused)
  • Refactor in RoadShaderPixelShader
  • ww3d2 MAX_TEXTURE_STAGES from 2 to 8

TODO

  • Add id's to pulls
