Fix socket leak and improve web terminal lifecycle management
- Remove double SSH connect() call in select_server (bug fix)
- Remove ui.timer polling - replaced with WebSocket-driven header updates
- Add open_terminal MCP tool - closes existing tabs, opens fresh terminal
- Add session_superseded WebSocket message - closes stale browser tabs on server switch
- Add connection_update WebSocket message - updates header with current server name
- Add separator line in terminal on server switch for visual clarity
- Reopen terminal automatically when tab is closed mid-session
- Send newline after terminal reopen to restore fresh prompt
- Guard open_terminal against calling when not connected
