OpenMayhem Core 0.2.244

Streaming providers now classify a provisional tool call that fails final
validation as request-specific model output. The gateway reports the existing
provider_model_output_invalid error without cooling or withdrawing a healthy
provider route.

Contract version 26 and its authenticated history are unchanged.
